What is MyChart?
MyChart is a medical record application system that will bring many new benefits to you, the patient.
This is an easy-to-use secure application that gives you access to different assets such as:
- Your health information
- A view your medications, test results, health summary and immunizations
- A way to request prescription renewals
- The ability to communicate with your care team online

How to Sign up for MyChart
**Please Note: If you are a patient of CommUnityCare, you DO NOT have to create a new MyChart account to access your Central Health information. Please use your existing CommUnityCare MyChart login.**
- Go to https://mychart.centralhealth.net/CentralHealth/Authentication/Login?lang=en-US
- Click the “Sign up now” button
- Fill in the required details and hit “Submit”
- Shortly, you’ll receive an Activation Code via email
- Enter your Activation Code into the MyChart page and click “Next”
- Your Electronic Health Record (EHR) account login includes an option for two-factor authentication. This adds an extra layer of security to prevent unauthorized access. If you want to enable two-factor authentication, click Continue and follow the next steps; otherwise, skip to Step 8.
- Now, update your contact details for two-factor authentication and click Continue. You’ll need a security code to verify your identity. Choose how you’d like to receive the code—email is usually faster than a text message. After you receive the code, enter it and click Next.
- Finally, create your username and password. You’re all set. When you log in for the first time, be sure to accept the Terms & Conditions and update your contact preferences.
- To use your account on an electronic device, download the MyChart app from the Apple App Store or Google Play Store. Sign in to your account and choose Central Health as your provider in the app.

- Appointment Scheduling – Schedule an appointment or request to have the clinic contact you.
- Medication Refills – Ask for a prescription renewal and receive notification when your medications are ready to be picked up at the pharmacy.
- Messaging – Send a message, review test results, talk face-to-face over video, or arrange to follow up with a doctor in person, depending on the level of care you need.
- Video Visits – Meet with a doctor over video to receive care from the convenience of your home or work.
- Test Results – Receive timely notification of results for tests you’ve had done and view test results from the past.
- Preventative Care Reminders – Keep track of routine care that is due and receive reminders for flu shots and other immunizations, physical exams and preventive care procedures.
- Health Summary – View a summary of your current health issues, medications, allergies, immunizations and preventive care all in one place.
- Friends and Family Access – Take care of your children and other family members all from one account.
- Visit Summaries – View, download, or send a copy of specific visit summaries.
- Pay Online – View billing statements and make online payments for your medical bills or visit copays through your bank account or credit card.
