Southeast Health & Wellness Center

The Southeast Health and Wellness Center (SEHWC) is a collaborative effort between the Southeast Austin community (Dove Springs, Del Valle and Montopolis), Central Health, and CommUnityCare.  This new person-centered health center serves the traditionally medically underserved areas of Southeast Austin and surrounding communities. The goal is to provide the full spectrum of primary, specialty and behavioral health care and offer supporting wellness programs and activities.   

In 2013 Central Health  launched an ongoing community engagement program to integrate the needs and wishes of the residents in the surrounding communities into the design of the facility. Over the course of the following two years, and continuing to this day, the relationships forged with those residents have driven the process to develop the wide range of services offered by the SEHWC.

The SEHWC is located at 2901 Montopolis Drive, previously home to the Veterans Administration Austin Outpatient Clinic.  Central Health purchased the property in 2011 and construction on the site began in March 2014. 

The first phase of the Center opened on October 13, 2014 with nearly 30,000 sq ft of health care clinic space operated by CommUnityCare, Central Health’s affiliated Federally Qualified Health Center system.  The clinic currently provides dental, family medicine, specialty care services (pulmonology and general gastroenterology), psychiatry and a pharmacy. 

The second phase of the Center, due to open in the Fall of 2015, will add an additional 37,000 sq ft of clinic and wellness program activity space and include expanded primary and specialty services, radiology, laboratory, a health navigation center, and supporting wellness programs and activities.  

Designed to serve our target population of uninsured and underinsured residents of Travis County, the SEHWC is open to all members of the community. It will serve as a site for several 1115 Medicaid Waiver Delivery System Reform Incentive Payments (DSRIP) projects including patient centered medical home (PCMH), behavioral health, specialty care, dental, and navigation.
